Rust spot on hood

The car has a few rust spots on the hood like this one that seem to have been covered up by touch up base coat. What should I do? Sand off the rust and re-touchup?

 

Isay this dependon how bad the rust is. If you can get under touchup and remove it and the rust is not bad, then yes you have to sand all the rust out - they say rust is like disease get it all out or it will plague.

From picture it really look like you have to have repaint, it almost look like rust is spreading worse than just under the touch up.

 

yea its definitely bubbling up the paint
